In this interactive session, palliative care specialists from John Muir Health will guide you through an interactive, engaging conversation on essential topics for planning our care as we age. We'll discuss the importance of having a legally designated decision-maker in case you can't speak for yourself, and how palliative care can enhance quality of life. We'll preview our new John Muir Health Older Adult Care Center and how it supports both patients and caregivers.
You'll also learn about Advance Directives and POLST (Physician Orders for Life-Sustaining Treatment) forms, how these documents supplement the conversations you have with family and caregivers. This session is designed to equip you with the knowledge and tools to make confident decisions for yourself or your loved ones, ensuring peace of mind if your health needs change. Come with your questions for a collaborative and supportive conversation.
